If your insurance requires a referral:
You may have your Primary Care Physician fill out a consult request form & fax it along with a completed medication list, pertinent office notes, EMG’s, radiology, MRI/CT reports, & a copy of your health insurance card to 207-942-5631.
If your insurance does not require a referral:
You may contact the secretary of the provider you wish to see. Our office will need to obtain any pertinent office notes, EMG’s, radiology, MRI/CT reports for the physician to review. You can have them faxed to 207-942-5631 or mailed to the address below.
If you do not wish to see a specific physician, please contact our referral department at 207-307-8948.

Canceling your appointment
If you are unable to keep your scheduled appointment please contact the office at least 24 hours in advance. This courtesy allows us to be timely and efficient in scheduling other patients.
It is recommended you arrive 15 minutes prior to your scheduled appointment time to allow us enough time to complete your paperwork. For your first appointment, we may need more in-depth information regarding your health history and please bring any x-rays or pertinent information related to your condition.
